How to Use this Benefit
To access the lab & imaging discounts you will need to call our provider network and pre-pay for the test you want performed. Prior to calling, be sure to write down the NuLegacy Lab & Imaging Group number which is “NL”.
IMPORTANT: Have your payment information ready. Acceptable methods of payment include Credit Cards (VISA, MasterCard, Discover or American Express) or Check by Phone.
This program is not currently available for use in the state of Illinois.
Step 1: Call the Lab & Imaging Customer Service department toll free at 877-395-8378.
Step 2: Give the Customer Service representative the following information:
1. NuLegacy’s group ID code = NL100410
2. The specific name of your desired diagnostic test
3. Your doctor’s name and address
4. Your payment information
Step 3: Write down your scheduled appointment time (if applicable) and the location of the testing center.
Step 4: Go to your appointment. Your test results will be forwarded to your chosen destination.
